1. Antibacterial Mechanism: Benzylpenicillin works by interfering with the synthesis of the bacterial cell wall. Bacterial cell walls are composed of peptidoglycan layers that provide structural support and protection to the cells. Benzylpenicillin binds to and inhibits the activity of enzymes called penicillin-binding proteins (PBPs), which are responsible for cross-linking the peptidoglycan chains. By blocking this cross-linking process, the cell wall becomes weakened and unable to withstand the internal osmotic pressure of the bacterial cell. As a result, the cell swells and eventually bursts, leading to the death of the bacterium.
2. Spectrum of Activity: Benzylpenicillin is effective against a wide range of Gram-positive bacteria, including Streptococcus pneumoniae, Streptococcus pyogenes, Staphylococcus aureus (sensitive strains), and many anaerobic bacteria such as Clostridium species. It also has some activity against certain Gram-negative cocci, like Neisseria gonorrhoeae and Neisseria meningitidis. However, many Gram-negative bacteria are resistant to benzylpenicillin due to the presence of an outer membrane that restricts the access of the antibiotic to the cell wall and the production of beta-lactamases, enzymes that can break down the beta-lactam ring of benzylpenicillin.

1. Prevention of Rheumatic Fever: In individuals who have had a previous episode of rheumatic fever, benzylpenicillin may be given prophylactically to prevent recurrent streptococcal infections, which can trigger further episodes of rheumatic fever and damage to the heart valves.
2. Prophylaxis in Surgical Procedures: It may be used before certain surgical procedures, especially those involving the upper respiratory tract, mouth, or gastrointestinal tract, to prevent surgical site infections caused by susceptible bacteria.
