Cladribine is a purine nucleoside analogue drug that acts as an antineoplastic and immunosuppressive agent. It is structurally similar to adenine and incorporates into DNA during cell division, leading to disruptions in DNA synthesis and function, ultimately resulting in cell death. Cladribine is specifically designed to target and eliminate rapidly dividing cells, such as those found in certain cancers and immune system disorders.
Treatment of Leukemia: Cladribine is primarily used in the treatment of hairy cell leukemia, a rare type of chronic B-cell leukemia. It effectively targets and eliminates the leukemic cells, providing significant clinical improvement.
Management of Multiple Sclerosis: In recent years, cladribine has been investigated and approved for use in certain countries for the treatment of relapsing-remitting multiple sclerosis. It reduces the frequency and severity of relapses by modulating the immune system and decreasing the activity of immune cells that attack the central nervous system.
Inhibition of DNA Synthesis: By mimicking adenine, cladribine is incorporated into the DNA of dividing cells. This incorporation leads to chain termination and inhibition of DNA synthesis, which disrupts cell growth and proliferation.
Induction of Apoptosis: The incorporation of cladribine into DNA also triggers the apoptotic pathway, a programmed cell death mechanism. This results in the elimination of targeted cells, such as leukemic cells or overactive immune cells, without harming surrounding healthy tissue.
Immunomodulatory Effects: In addition to its direct cytotoxic effects, cladribine also modulates the immune system by reducing the number and activity of certain immune cells. This makes it an effective treatment for autoimmune disorders like multiple sclerosis, where the immune system attacks healthy tissue.
