Cilostazol is a selective inhibitor of phosphodiesterase 3 (PDE3), primarily targeting PDE3A in platelets and vascular smooth muscle cells. By inhibiting PDE3, it increases intracellular cyclic adenosine monophosphate (cAMP) levels, leading to vasodilation and antiplatelet aggregation effects. It is chemically classified as a 2-oxoquinoline derivative and is used in clinical practice to improve symptoms of peripheral arterial disease (PAD) and prevent secondary stroke.

 

1. Peripheral Arterial Disease (PAD): Cilostazol is indicated for treating intermittent claudication (IC) in patients with PAD, improving walking distance, and reducing symptoms like pain and cramping during physical activity.
